I am the competitor and, or the person responsible for the competitor set out in this application.
I am the owner of the horse/pony(s) entered or his/her agent and have been authorised by the owner to make this entry.
I have read the terms and conditions for ESNZ & NZPH Star Spotters Scholarship and I/my child and horse or pony are eligible under the information listed under DOCUMENTS tab of this online entry.
I consent in terms of the Privacy Act 1993 to the information on this form being used by the organising committee and by Equestrian Sports New Zealand for all purposes connected with the administration of equestrian sport in New Zealand.
If successful, I give permission for my/my child's photos and updates to be used for promotional purposes on the ESNZ and NZPH websites and Facebook pages during the season.
I understand that the committee of our ESNZ Area Showjumping Group are not obliged to accept all or any nomination and that any decision on who is to be the successful applicant for the Area is final.
